How will Angela store your information?
- Email: Angela will securely store your email address and any email correspondence via her secure email files, which are only accessed via passwords.
- Paper: Angela writes session notes for each client during the first 2 sessions. All notes from sessions are handwritten by Angela. These notes are filed securely in a locked cabinet. Sessions 3 onwards are written and filed slightly differently. Angela uses shorthand for these notes and no identifying information is written on these forms - only initials and the date along with what the session entailed and which psychotherapeutic avenues we explored or used. These note are then filed in a locked cabinet in date order, rather than alphabetical. Three years after termination of therapy, Angela will destroy all these notes by burning or shredding them herself. The only notes Angela types up from sessions and stores either via email or Word documents, are those requested by the client to send to psychiatrists or GPs as referrals. Angela will always seek client approval before doing this and the client will be shown the information Angela intends to pass to a psychiatrist or GP before sending.
- Laptops / Computers: Angela stores initial email correspondence on computer which are secured via passwords, which are changed at intervals. The device Angela uses has a virus checker. No other person accesses or uses this device, only Angela.
- Mobile Phone: Angela may store your mobile phone number, and any texts you may have sent her, along with your first name and an initial for your surname. She will store this for the duration of your time together and after a year, will delete your number from her phone. This phone is the personal property of Angela and used by Angela only.
- Website: In the back office of Angela's websites, there is a list of client contact details from those clients who've emailed Angela through those websites. This back office is password protected, as are the websites.
- Appointment system: Angela uses a professional online appointments system which stores client appointments electronically. The online diary is protected by password and notes the clients' appointment times, their first and last name, their email address and their mobile number. Only Angela has access to the online appointment detail and information regarding clients.
- Records of payments and invoices: Angela's online appointment system also records payments and invoices. Any bank details or payment card details are not stored in Angela's systems. Only Angela has access to the invoicing system.
- CCTV on Clinic Premises
There is CCTV operating on Angela's clinic premises - outside the clinic and not inside. This is for security purposes only and the prevention of crime. The data is held in the "Cloud" for 5 days and then erased.
- Will Angela share your personal information?
Angela does not sell, loan, rent or share personal data with any third parties. Your contact details and personal information will never be shared with outside parties apart from the following exemptions:
Clinical Supervision
As a Member of the BACP (British Association for Counselling and Psychotherapy) Angela complies with the requirement to attend regular (monthly) supervision with a qualified psychotherapy supervisor. Aspects of some client cases may be discussed but under no circumstances will your full name, contact details or any other identifying information or detail be shared or revealed to her supervisor.
Therapeutic Will
Angela has a "Therapeutic Will". This means, in the event of death, illness or hospitalisation, she has appointed an Executor to make contact with current clients to inform them of the situation and advise as to the options. This Executor has no prior access to client contact detail. In the case of death, the appointed Executor has instructions to personally burn or shred all written client notes and delete from Angela's personal computer all client contact details.
Confidentiality
It is highly unusual for a psychotherapist to have to break client confidentiality. There are guidelines as to when that may happen and Angela adheres to these guidelines. If Angela believes her client's or another person's safety is at risk (e.g suicide or violence), she may contact a suitable authority (e.g GP). Angela will, in the first instance, discuss this with you to seek your permission. However, in rare circumstances, such as terrorism, Angela will do so without your permission. Angela is required by Law to pass on information relating to acts of terrorism and ongoing abuse of children or the vulnerable.
